I've been both a test-taker and a VE (volunteer examiner), and formerly accredited by two VECs (ARRL and W5YI). There is an entirely open question pool, posted and reviewed every $n$ years. One of the complaints about the system (when I took it, over 10 years ago) is that you could cram just the exam questions, instead of showing "true learning".
