multiple edits per user per file

This patch should fix the problems with client/server commit not unediting.

Rob, can you also verify that it works for NT?  Thanks.

For those that've missed the previous email.  This patch allows the same user to
"cvs edit" the same file within different working directories without losing
prior edits.  It also includes some of the other stuff I've done (eg "cvs edit
-c" et al, ...).  The ChangeLog diff should list it all.
