Simple things: you're sending the output to a pipe, you're doing a join,
or the file is marked as binary.  More complicated things: someone
outdated (deleted) the revision you currently have or the revision
you're trying to update to, the revision you're trying to update to is
dead, or various internal errors occur when trying to generate the patch
(such as running out of disk space).
