Thanks for  your help  by sharing your experience.

Both the problems are solved and now recovering data.

In my case  hdd is using as external (usb to sata converter), so
whenever hdd is failed to respond then i use to reset power for hdd
and reconnecting usb . In this case system reset is not used.

Initially i was using UBANTU 8.04 and the avalible ddrescue  by
default. I downloaded 1.13 and installed it.( because i am new to
linux it took some time to do this.) In old version -T option was not
there but in 1.13 its available. It saved me alot . And next step
using  -e0 or e1  its beautiful  and really these options made
ddrescue is fully automated.
