Both the problems are solved and now recovering data. In my case hdd is using as external (usb to sata converter), so whenever hdd is failed to respond then i use to reset power for hdd and reconnecting usb . In this case system reset is not used. Initially i was using UBANTU 8.04 and the avalible ddrescue by default. I downloaded 1.13 and installed it.( because i am new to linux it took some time to do this.) In old version -T option was not there but in 1.13 its available. It saved me alot . And next step using -e0 or e1 its beautiful and really these options made ddrescue is fully automated.
