I had one case where the first error seemed to be passed up correctly and 
ddrescue saw the bad area, but from then onwards, ddrescue got a copy of the 
same data, over and over, with no errors being reported. Of course, I thought 
most of the disk was good and left it running.

I now use IDE/SATA and tend to favour direct mode after the first pass.
