I'm not going to use the patch from Alexander because its coding style
is too different from ddrescue's. For example, the patch uses macros,
which are not desired in ddrescue.
I'll also try to make the output format a little less redundant. Any
suggestion about this is welcome.

I am certainly interested in making ddrescue behave reasonably in all
circumstances. I only hope the algorithm does not get so complicated
that ddrescue becomes Skynet. :-D
