Currently, if the --frame option is specified, XHTML-1.0-Frameset is used in
all files.
XHTML-1.0-Frameset should be used only in the file containing <frame>, i.e.,
index.html.
Specification should be changed as follows.
if the --frame option is specified, then
generate XHTML-1.0-Frameset for index.html,
generate XHTML-1.0-Transitional for other files
else if config variable 'xhtml_version' is set to 1.1 then
generate XHTML-1.1
else
generate XHTML-1.0-Transitional
