The differences could be
1) different PDF-files, slide-demo.pdf
From "test-groff -Tps -mpdfmark -mom -Z ..."
and
from "test-groff -Tpdf -mom -Z ...":

To 1:
I use "test-groff" to create the files in a copy of the current groff
repository.
To 2:
Used pdf-viewers from the testing version of Debian.
xpdf bigger letter than with "gv" and more space in front of ')'
gv OK
atril more space in front of ')'
mupdf OK
zathura more space in front of ')'
