   Would it be useful to make it clear that new contributions and
   changes are licensed under the GPL, or is that not the intention?
   Adding the GPL license headers, and increment the copyright years
   there, would then be appropriate.

It would be useful and it is important to do so, and I'm kinda in the
process of doing that.  Only that the whole `when to add a year' issue
confuses me greatly.  The current state of all the headers and files
is quite bad.
