In the incipit to the `Repeats' section, I think it should be

Later in the chapter there is a snippet which demonstrates this.
Not sure. It would appear to me that
2 |
:|:
4 |
would be wrong, for example.
Hindemith, "Elementary Training", p. 50 & 70, has some examples. In
the former case, there is a courtesy time signature, two lines, like:
4/4 ... | 3/4 ... | 4/4
|: 4/4 ... | 3/4 ... | ... :[
In this case, it is necessary to put it inside. But it interesting to
add a time signature at the end of the line before it.
On p. 70, there is
4/2 ... |: ... |
... | ... |
______________
|1. |
... | 2/2 ... 4/2 :|
So there is an extra 4/2 so that the meter change tales place in the
repeat.
My guess us that the rule is that they can be put wherever there
results in a correct expansion, with some extra as desired for clarity.
