This issue should be fixed.
To test, copy the example from comment 10 into a file named "test.ly", and
run the command
lilypond -fpng -dresolution=100 test.ly
Verify that the image size has dimensions approximately equal to A4 paper
size. For me, GIMP reports the dimensions as 827 x 1169 pixels, which is
equivalent to 210.06 x 296.93 millimeters.
