I couldn't find any mention in Gardner Read. But Elaine Gould's Behind Bars
explicitly says: Tremolo strokes must stay within the stave so as not to be
confused with, nor collide with, a ledger line.
I'll attach the example from Gould as a .ly file and the wrong lilypond
output.
