Following the work by (mainly) Julien, make doc is now in pretty good
shape. Changing a documentation file and re-running make doc should
regenerate the document that uses that file. make doc clean should allow a
further make doc to be run satisfactorily.

does not acknowledge the improvements and should be updated.
