Side-fingering has always (at least since 2.12) been too close in notes a third apart; that's why I chose to show how to reduce the font-size in the LM (see towards the bottom ofhttp://www.lilypond.org/doc/v2.14/Documentation/learning/within_002dstaff-objects )
I've attached a cutting from the 2.12 LM pdf, which is just as bad as Mike's. So no regression. Not sure about the flag. The flag shape and stem length were changed some time ago; maybe this was a contributing factor. Trevor
