Same result in 2.14.2 and 2.15.35.

Haven't tested in 2.12.

I'm not sure if it's a regression, but it's ugly :(

Side-fingering has always (at least since 2.12) been too close in
notes a third apart; that's why I chose to show how to reduce the
font-size in the LM (see towards the bottom of
http://www.lilypond.org/doc/v2.14/Documentation/learning/within_002dstaff-objects )
I've attached a cutting from the 2.12 LM pdf, which is just
as bad as Mike's.  So no regression.

Not sure about the flag.  The flag shape and stem length were
changed some time ago; maybe this was a contributing factor.
