Hi Simon,

thanks for your report.

I reduced the code to a small example:

      \relative c' {
        d2. d8 d(
        \break
        f1)
      }
      \relative c' {
        c1
        \clef alto
        c1
      }
  >>

Some testings showed that it appeared somewhere between 2.15.40 and
2.16.2 (I don't have other versions between them any more)
Same with 2.17.12
