The whole index subentry thing has never really been implemented.  Don't
take any code or comments about it in any of the sources as anything but
random attempts at trying it that never came to fruition.

Of course, if you'd like to work on it and get it all straightened out
and implemented, that would be great.  Texindex, texinfo.tex, and
makeinfo all have to be modified.
