This is probably my fault. I sent in a patch a few months
ago to create the index along with a few @cindex entries.
I only looked at the HTML output. I did not even verify the
printed version. Shame on me.
Anyways, would adding "@setchapternewpage on" just before
"@node index" solve the problem?
