Some indices don't show up (was Re: backslashes, macros, indexes, texinfo)

I tried this in a test document, but for some reason I don't know, the
only indices that are appearing are the function and variable indices.
The others give "(Index is non-existent.)" The output of texi2dvi is
below.

The index files all appear to be created properly:
