provider: debitel (contracted)
age: approx. 12 months
network freq: 900 MHz

I have a 71234(O)/4022. This is the only non-ancient SIM card that works
at all on the AT&T network in the USA. The network registration is spotty.
At times the phone registers quickly at boot, at other times it doesn't at
all. Sometimes a reboot makes it register. This is all without touching
the SIM. This instability, in addition to the fact that the pins impact
dead-center on the pads of the SIM makes me think that the wiggling of the
SIM is a red herring. I'm including several runs of the diagnostic script.
These were done one right after another, without moving or touching the
SIM. After the first boot, the FR said "Registering..." the whole time,
even though the logs indicate that registration was acquired at one point.
Then I reboot, and the FR registered successfully. Several noteworthy
points are that once registered, the logs indicate that the phone kept
changing the network cell it was connected to. Also, one of the commands
generates a number very similar but not exactly equal to the code of my
SIM (something like 7 digits in a row match). I don't know if this is
supposed to match my SIM code exactly or not.

I have added my gsm_log output produced with my O2 (germany) sim card,
hope that helps.
Having become impatient, I ordered a new sim card from O2. The geniuses
sent me a new card of the exact same type. Needless to say it shows the
exact same behaviour as the other two cards of that type that i already
have :(
Bottom line: At least in some countries and with some providers, the
problematic cards seem to be more common than some people (including me)
hoped for.

Provider: O2 Germany (for picture see above)
I have made the same experience as oneiros, got a new SIM and it was one
of the same type I had before. Hope that log helps to track down and kill
that nasty problem :/
