Right.  Apache does not allow line ending comments (see
http://httpd.apache.org/docs-2.0/configuring.html )
so it is interpreting that as
"deny the IP address 123.11.11.11, the hostname "#" and
the hostname "domain.com"."  It does a lookup to match against
those hostnames.

You should also have a warning in your error log telling
you to "use quotes around "#" or something like that.
