Besides using the prefork MPM on FreeBSD, you need to make absolutely sure that
APR is not built with thread support.  I believe that as long as APR_HAS_THREADS
is set to 0 in apr.h you should be okay.  Even prefork exhibits strange behavior
when thread support (libc_r?) is linked in.

If the use of thread libraries is not the problem:

Please use lsof or some other tool with similar capabilities to see what file
descriptors are actually open by the process at the time that you see such 
errors.
