Can't specify filter order when combining AddOutputFilter and SetOutputFilter

The above analysis is incorrect.  Here's the result if sub.html is expanded to
three lines:
     1  <html><head><title>Title</title></head><body>
     2  <h1>Start</h1>
     3  <h3>[     1     This is the start of the include.
     4       2  This is the middle of the include.
     5       3  This is the end of the include.
     6  ]</h3>
     7  <h1>End</h1>
     8  </body></html>
>From this one can see that the filter is being called on the combined final
document (which is perfect), as well as on the file fragment (which is
redundant).  So all we need to do is stop it from filtering the fragments.
