HeaderName and ReadmeName fail silently when set to file without proper MIME 
type

I think index.head should be included as text/plain in your case (assuming the
default mime type is set to text/plain as it is by default, and content
negotiation is enabled)

Another problem I had was that the online documentation says that if set to
"HeaderName README" then if README.html is not found it will fall back to README
Actually, it falls back to README.txt (As correctly documented in a comment in
httpd.conf)
