When using http://[3ffe:b00:4041:abcd:202:55ff:fead:15e9]
(insert any IPv6 address) the SERVER_NAME environment variable is set to
         3ffe:b00:4041:abcd:202:55ff:fead:15e9
It should be set to
         [3ffe:b00:4041:abcd:202:55ff:fead:15e9]

Note that httpd.conf has the following configuration option:
         UseCanonicalName off
