Ok, I have a few corrections.

1) I've gotten this bug to work on a Linux server now, so I've marked the OS 
as All.
2) The file that needs to be in the folder appears to be HEADER.html (notice 
the caps). This had not been noticed by myself earlier due to Windows not 
making the case distinction that Linux does.

An example is located at: http://gideon.theenvoygroup.net/apachegroup/. Also, 
this bug has been seen by myself in version 2.0.47 of the Windows build, but I 
have not had a chance to test it with other versions.

Lastly, in a quick test on Apache 1.3.27, this bug does not occur on that 
version (a custom RPM build was used on Linux).
