REQUEST_URI and HTTP_REFERER don't handle URL/URI anchor/hash, i.e. '#name'

The anchor is called fragment and not part of the URI. It may be part of an URI
reference. Anyway, typcially it isn't sent to the server at all, so there's no
way to deal with it.
