apr_pool_clear fails if the cleanup handler is still running

The bug here remains that the prefork MPM calls async-signal-unsafe pool 
functions from a signal handler; so really, the only place to solve this in the
MPM, maybe using Colm's "graceful shutdown" plan.  (applies to worker too, I 
guess)
