I get the following error in the rewrite log (with logging set to 2): (2) uri already rewritten. Status 1, Uri /x/y/z.jpg, r->filename (null) In the error log I get: Parent: child process exited with status 3221225477 -- Restarting. This is indeed an access violation error number in Windows. I only have memory cache enabled, not disk cache - so it does possible this is actually an error in mod_mem_cache.
