It can occur if an SSL connection times out or is forcibly closed by the client (i.e. under normal operating conditions). If you are using a reverse proxy, then you may be suffering from bug 34452 -- use http://people.apache.org/~jorton/httpd-2.0.52-sslbuff.patch to fix that.
