Version 2.2.3 and 2.2.4 will give the error
"Request Range Not Satisfiable" about 70% of the time.  (The larger
the PDF file the more often it will happen.)  This seems to happen only when
a large PDF file is requested by the client (Firefox 2.0.0.2 or IE6SP1
with Adobe plug-in 7.0.9 or 8.0).  Version 2.0.59 works perfectly.
It looks like version 2.0.40 had a fix for the PDF byterange failtures,
but it's not sure whether that's related to this or not.
Maybe the fix in the 2.0.x series didn't get ported to 2.2.x???

This problem is reproducible.  A 6MB PDF file was used for this testing.
It makes sense that the larger the PDF file the more likely the client
has to do a byterange request, hence the problem with large PDF files.
