The preferred way to enumerate pci devices is to use `pcidump' (and
`usbdevs' to list usb devices), rather than 3rd-party software.

That said, your card does not even appear as `not configured' in your
dmesg (the three PCI slots of the Ultra 5 are on pci2, which is empty on
your system).

It is possible that your card requires strict PCI 2.1 compliance; that
would explain why it isn't detected at all (Ultra 5 systems do not
support strict PCI 2.1 devices).
