Began occurring immediately after the upgrade sets were installed and the 
system was automatically rebooted. Shutting down the machine manually and 
powering it back on produces the same result.

I see the following:

Tried using set device with sd0a, sd0c, sd1a, sd1c, hd0a, hd0c, hd1a, hd1c as 
arguments - boot failed with all of them.
