I've revisited this and moved `rtems_iodev_destroy` and 
`rtems_iodev_free_and_destroy` to a single call, 
`rtems_iodev_destroy_unregistered`. Additionally, a failed register does not 
destroy the `iodev`. This makes the API much clearer as the implied 
symmetricality of the API is removed.
