"A Practitioner's Guide to Software Test Design" by Lee CopeLand.

Paul Herring, one of this group's moderators, runs another group called 
'softwarequality' (http://tech.groups.yahoo.com/group/softwarequality/). 
  You should join that group and look in the archives and Files/Links 
sections.  LOTS of resources (including books).  The group itself isn't 
very active but it is a good resource.
