I checked your issue but it seems working properly.
I have created an Evaluation in Human Resources/Evaluations/Evaluations and
choose the employee "Quentin Paolino" and choose the evaluation plan
"Manager's Evaluation Plan". Then, start the evaluation (button). you get three
phase because the manager of Quentin Paolino is Antoine philippe.
i describe you in detail
first phase of plan is "send subordinate" and "action is "bottom up" so for
bottom up state, user working under antoine is add on interviewer.
For second phase of plan is "send manager" and "action is "top down" so
for Top down state, parent user(manager) of the employee is add on
interviewer.
For third phase of plan is "send to employee" and "action is "self
appraisal request" so for "self appraisal request" state, the current
employee's user is added as interviewer.
For fourth phase of plan is "final interview with manager" and "action
is "Final interview" so for self "appraisal request" state, parent user
is add on interviewer.

In trunk, web, hr_evaluation :
In evaluation (from Human Resources/Evaluations/Evaluations), the m2m
"Interview Evaluation" don't take account phases specified in the evaluation
plan.
Install the module with demo data. If you check, there are 4 phases in the
Evaluation plan (Manager's Evaluation Plan from Human
Resources/Configuration/Periodic Evaluations/Evaluation Plans). So, create an
Evaluation in Human Resources/Evaluations/Evaluations and choose the evaluation
plan "Manager's Evaluation Plan". Then, start the evaluation (button). In the
m2m "Interview Evaluation", phases appear but not 4, only 1 or 2.
