shouldn't be able to add an attachment from the "add an internat note"

Bug description:
In CRM cases, in history tab, there is an "Add an internal note". this
pop-up allows to add an attachment. this feature doesn't work, it
takes too much time (more that 10 minutes, i stopped the loading).
this feature is useless there because the user can add an attachment
from the right bar. So, remove this tab from the pop up (add an
internal note).
