Title:
hr_evaluation : survey of employee is not able to being done and
allows unlimited no. of time to answer

Bug description:
when i answered a survey for evaluation and complete survey it should be in
done state and should not allow me answer again
but after completing survey it does not change the state and allowing me to
answer again and again.
Steps:
(1) Human Resource > Appraisal > Appraisal:
create new appraisal plan for employee assign particular plan
(2)Start appraisal
(3)it will create appraisal line , select one record
(4)there is one button "interview question", complete a survey step by step ,
done
