I just wanted to close this issue.  It turns out this was an oddity with 
the system BIOS.  When verifying the usb image contained the 
modifications I made to menu.lst I removed the usb stick from the 
system.  When doing this the BIOS must have seen that the usb (hard 
drive) was not present and removed it from the hard drive boot order 
list.  So upon replacement of the usb stick it was not booting from the 
usb image itself but from the same image contained in the cdrom drive.
