Its not strictly needed in the first entry, its depends on what
you need.

The first one is the one that is selected by default when the 30
seconds times out at the grub menu prompt.  So if you are in an
environment where you expect just to be able to power cycle your
system, and for it to boot into AI and automatically do the install,
then you want to modify the first entry.

But if not, then you can just explicitly select the second entry
when at the system console, when it boots the AI image.

I have edited the clients menu.lst file and added livessh=enable and 
livemode=text .  Are these still required?

Use livessh=enable if you want to enable ssh on the client so that
you can remotely log into it while its installaing.

livemode=text is not needed anymore.


Note, you can use the -b option to create-client to add these to the
menu for you.
