Just for info if anyone finds this by search: I had it happen again, and fixed it by sending a restart to openbox. There must be a bug in openbox (3.5.2) where it loses track of application windows.
An additional symptom is that the Cairo-Dock Switcher, which normally shows a map of each virtual desktop with the icons on the correct sides, was showing all stacked on the left, as if they were all on Monitor 0. It turned out that cairo-dock believes that it overlaps if a window is maximised on the *other* monitor. If the window is dragged to maximum size, rather than maximised, there is no problem.
