I believe that happens when you forget to include the prototype or
scriptaculous javascript files in your code.
Add the following lines before the </head> tag of your default.thtml
layout
<?php echo $javascript->link('prototype') ?>
<?php echo $javascript->link('scriptaculous.js?load=effects') ?>
