One of the troubleshooting steps I take whenever I have this error on a project is to log onto the database using the same connection info as in my database.php file. The screen shot of MySQLWorkbench indicates a connection using 'root', which can see all the tables in all the databases; the screen shot of the PHPMyAdmin session seems to also be from the 'root' user perspective. It might be as simple as granting user fonyk_db insert/update/delete privileges to the acos table in the fonyk_db schema.
