I go this tid-bit and it works. But I would love to tweek this layout so
that everything aligns just the way I want to.

The above generates this code:
<div class="input text">
<label for="ProviderLastName">Doctor Last Name</label>
<input id="ProviderLastName" class="input_style" type="text" maxlength="100"
name="data[Provider][last_name]">
</div>
<div class="submit">
<input class="submit_style" type="submit" value="Search">
</div>
The 2nd div I could do without. Any pointers? The book doesn't seem to
have a section dedicated to this, data-mining google for the moment.
