A Cake based accounting package would be great. It is something that I too have been thinking about. There are plenty of small business (MYOB, Quickbooks, MS Money etc) packages, and plenty of large business solutions (SAP Financials etc), but few if any viable options in the middle. And certainly none that are free (that I can find).
It is one of those projects that I would love to do one day, but it would take some serious commitment to keep the accounting rules straight for each country. That does however suggest an interesting pairing of open source and commercial support. There would be a real business there for a commercial vendor to do the customisation of tax/accounting rules for their country. Given that they wouldn't be maintaining the entire package it would work out cheaper than the fully commercial accounting software. Sigh. It would be nice to have.
