Go to
C:\Program Files\Adobe\Adobe Dreamweaver CS3\configuration
\DocumentTypes\MMDocumentTypes.xml
and search for:
winfileextension="php,php3,php4,php5"
and modify to:
winfileextension="php,php3,php4,php5,thtml,ctp"
