Right after I posted this I re-read the Model section, and found the
way to solve the problem.
$this->Item->ItemList->save($this->data);
$itemListId = $this->Item->ItemList->getLastInsertId();
$this->data['Item']['item_list_id'] = $itemListId;
This both creates the new list and associates it to the new item.
