What is your setup? What is your server docroot, where is your app folder located and where is the cake folder?
I regularly have my development projects running as subfolders on my machine with no problems with rewrites. Problems with rewrites usually come from non-standard installs, dodgy apache configurations or incorrect paths in index.php.
