Since I only ever implement someone else's designs, I just print out their design and go through each section and link and write notes about how I will implement - ie. what controller & action it should be. Jot down anything unusual or special. Then I create my database based on information displayed in those designs, and then bake my models, controllers and views and start hacking away.
