Ahh truns out it's simple ....
Change .htaccess from:
<IfModule mod_rewrite.c>
RewriteEngine on
RewriteRule ^$ app/webroot/ [L]
RewriteRule (.*) app/webroot/$1 [L]
</IfModule>
To:
RewriteRule ^$ app/webroot/ [L]
RewriteRule (.*) app/webroot/$1 [L]
Seems an MT GS doesn't like:
RewriteEngine on
