I found there is newline character "\r\n" in the start of every page no matter which layout I am using.
i.e. before "<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1......." I have also checked that all layour .ctp do not contain "\r\n" at the start. This make my captcha image cannot be shown properly.
